At Harmony Healthcare Long Island, your wellness is our primary concern, not how you will be paying for your health care services. As part of our mission, we provide affordable, quality medical services to anyone who walks in the door of one of our health centers.

How to Pay for your Care

If you DON’T have Insurance

If you would like assistance with applying for health insurance, you will need:

  • Proof of income: for you and your spouse – 2-4 current pay stubs or notarized letter from employer, disability statement etc.

Those who don’t have any type of insurance coverage can still receive medical services at Harmony Healthcare Long Island. Many of our patients pay based on our sliding fee scale.

If you HAVE Insurance

Please bring these items:

  • Health insurance card
  • Co-payment (if you have one)

You may have to submit co-payments or deductibles. We accept cash, credit and check payments.

We also accept managed care plans and public health insurance, including Medicare, Medicaid and Child Health Plus.

How our Sliding Fee Scale Works

Our sliding fee scale is available to all income-eligible insured, uninsured and under-insured Long Islanders.

You can apply for our sliding fee scale program when you come in for a visit. Or, you can download, print and fill out the application form ahead of time.

To be eligible for the sliding fee discount program, you are required to have acceptable proof of income. If you qualify, the discount can apply to all of our health care services, except those provided outside of Harmony Healthcare Long Island locations.

Child Health Plus  (CHP)

To be eligible for either Children’s Medicaid or Child Health Plus, children must be under the age of 19 and be residents of New York State. Whether a child qualifies for Children’s Medicaid or Child Health Plus depends on gross family income.

In general, all children who are not eligible for Medicaid are eligible for CHP. Families with incomes below 160% of the Federal Poverty Level (FPL) can receive free insurance for their children; families with incomes over that pay on a sliding scale from about $9-$60/month per child.

Those families with higher incomes will need to pay a monthly premium of $9, $15, $30, $45, or $60 per child per month, depending on family income and family size. Those families whose incomes are 400% above the FPL are not State subsidized and will need to pay the plan premium rate.

As of January 2019, 32 states cover parents and other adults with incomes up to 138% FPL ($29,435 per year for a family of three and $17,236 per year for an individual in 2019).

Paying for your health care is easy with us

Family Health Plus (FHP)

Family Health Plus is a public health insurance program for adults who are aged 19 to 64 who have income too high to qualify for Medicaid. There are minimal co-payments for some Family Health Plus services. Health care is provided through participating managed care plans in your area.

Affordable Health Care Services Available to All

We provide health services to individuals and families with and without medical or dental insurance.

If you are uninsured, you may be eligible to receive reduced rates based on our sliding fee scale. The discounts available are typically based on family size and income level.

You will never be denied medical services if you are having difficulty paying for your health care. We provide health care to all regardless of insurance status or ability to pay.

Pharmacy Services

Genoa Pharmacy is located at our 380 Nassau Road, Roosevelt location. The HHLI pharmacy services are available to HHLI patients ONLY with RX pick up or mailing options (delivery options TBD). Genoa pharmacy participates in the 340b discount prescription program for eligible patients.
