WIC is a federal supplemental nutritional food program providing breastfeeding support, nutritional education and benefits to those that qualify. The New York State WIC Program has many partners in the community and has worked hard to develop relationships with a variety of other social service programs (e.g. Food stamps, Headstart, Medicaid, etc.). The LIFQHC WIC program continues to build a strong referral network to support and encourage development and stability to the participants we serve.
Applying for the New York WIC program is a fairly easy process. All you need to do is to locate and call your nearest LIFQHC WIC office. For locations and hours please click here. By doing this you will able to schedule an appointment with a LIFQHC WIC program representative.
Before going to your appointment you must remember to carry the important documents, which can be found by clicking here.
You also need to download, print and fill out the relevant forms available by clicking:
During your appointment LIFQHC WIC staff will verify your documents and ask you questions directed at establishing your eligibility to the program. Upon successful completion of the process you will start receiving the benefits of the New York WIC program.
